With the advent of the Affordable Care Act (ACA; a.k.a. Obamacare) and existing parity legislation, addictions, or severe substance use disorders, will be considered as other chronic health conditions. Treatment outcomes – will be newly defined and closely watched. Reimbursement models will also shift to motivate changes in treatment methods and models under the significant financial pressures to improve the overall efficiency and effectiveness of care provided. Increasing attention will be paid to the use of best practices and evidence-based treatment. These changes will alter treatment management and reimbursement paradigms. The behavioral health research community, standards-setting bodies, clinical managers, payers, and patients will each have role in defining what is meant by the core concepts that will play a critical role in determining patient treatment modalities and reimbursement approaches. Terms such as “medically necessary treatment”, outcomes-informed treatments, and even “behavioral health” itself, will all be forced to adhere to more stringent and precise meanings. These definitions will define the scope, intensity, duration and quality of SUD treatment. But it is far from clear whether these terms will be defined with rigor and precision. And just who will influence (dictate) the definition of these terms? Specifically, will the definition of what treatments are deemed “medical necessary” be defined by payers, pharmaceutical companies, or behavioral health researchers and clinicians? And just what constitutes an “outcome?”
